What We Collect
Information You Give Us
The contact form on this site asks for your first and last name, your email address, your phone number, an optional preferred office, and your message.
Information Collected Automatically
Like most websites, this one records basic technical information when you visit: the pages you view, roughly how long you spend on them, the site that referred you, your browser and device type, and your approximate location based on your IP address. This comes from Google Analytics and Cloudflare, and it is reported to us in aggregate rather than as a profile of you.

How the Contact Form Works
When you submit the form, your message is sent to our office as an email. It is not saved to a database, and it is not stored on this website. There is no record of your submission on the site after the email is sent.
Cloudflare Turnstile checks that the submission comes from a person rather than a bot. That check looks at the request, not at what you wrote.
Because email is not a secure channel, please do not use this form for detailed medical information. Give us a brief reason for your visit and we will collect the rest by phone or at your appointment.
How We Use Information
We use what you send us to respond to you, schedule and confirm appointments, and answer your questions. We use the automatic information to understand how the site is used and to keep it working and secure. We also use it where the law requires.
How We Share Information
We do not sell your information. We do not trade it. We do not share it with advertisers.
Information is shared only in these situations:
- Service providers. Google Analytics and Cloudflare receive the technical information described above in the course of providing their services. Google receives the fact that an appointment request was submitted, with no accompanying details: no name, email, phone number, or message.
- When the law requires it. In response to a valid legal request.
- To protect safety. Where disclosure is necessary to protect the safety of a patient, our staff, or the public.
How We Protect Information
This site is served over an encrypted connection, and contact form submissions travel encrypted from your browser to our office. Because the form stores nothing, there is no archive of submissions on this website to be exposed.
No method of transmission over the internet is completely secure, and we cannot guarantee absolute security.
Your Choices
- Ask what we have. Contact us and we will tell you what information we hold about you.
- Correct it. Tell us if something is wrong and we will fix it.
- Ask us to delete it. We will delete what we are not required to keep. Medical records are governed by law and by our Notice of Privacy Practices rather than by this policy.

This site links to other organizations, including hospitals, physician directories, and Google Maps. Once you follow a link you are on their site, under their privacy policy, not ours.
Children’s Privacy
